For example, if you've written your book in Word 2004, you can set the case
to 'title word' (i.e. Title Case, or to 'title sentence', i.e. Sentence
case) for any text range, and if you've used Word's Headings styles for your
titles and subtitles, you can easily get hold of just the ones (levels) you
want and change them. I would guess (without having them myself) that
scriptable publishing layout applications such as Quark and InDesign would
have something similar.
