I think your basic problem is that the Finder still doesn't know that
different files of the same type are supposed to have different icons.
Is your custom icon function getting called for every icon that appears
on the screen? It seems more like the Finder is caching the icon it
gets from you and re-uses it for other files of the same type even when
you don't want it to.
