It's not clear from the QuickLook doc precisely which clients ask for thumbnails and which ask for previews. I can guess, but it would be nice to know for sure.

Well, currently the only client that asks for a full preview is the QuickLook view in the Finder (when you hit the spacebar and actually bring up the QL previewer). There's no public API for 3rd party clients to even ask for a preview, so everyone else will ask for a thumbnail preview. That's what Default Folder X uses for its previews in Open and Save dialogs (I'd use the full preview if there was an API to get it, but Apple hasn't opened that up yet). That said, it'd be great if you actually returned a real preview when the image size is > 128 x 128, for apps like Default Folder X that will actually display it full-screen when the user wants to see a detailed preview.
