.xinitrc works fine and is the recommended way of doing it, I think.

And, generally speaking, how do I start a window manager under Darwin?
Do I need an .xinitrc in my home directory? /User/my_user_name/.xinitrc
or how does Darwin handle this?

I think what you want is a .Xclients file in your home directory that
contains something like this.

xterm &
exec afterstep

Of course you can replace afterstep with your wmoc.
