This is a known bug, listed in the Mac OS X 10.0 release notes at:

Today I was adding some file handling capability to my application. When the user quits the application, I want to pop up a dialog asking if the file should be saved before quitting. If the user quits by clicking the close box, this all works fine. However, if the user quits by choosing the Quit menu command installed by the system, the app hangs in some sort of deadlock after showing the dialog.
