This may be related: I found that with XCode the icon vanishes every time I "Clean All Targets". My fix has been to write a one-line shell script that says, roughly:

cd program_development_directory
cp MyIcon.icns build/Development/MyProgram.app/Contents/Resources

I run it every time I clean the targets, after I have recompiled. A kludge, but it seems to work.
