Some posix-type things read the file. I think one of them is lookup'd resolver (note the transaction when I change my dns server:

15:59:09 open private/var/run/resolv.conf 0.000041 lookupd 15:59:09 stat private/var/run/resolv.conf 0.000031 lookupd
15:59:09 open private/var/run/resolv.conf 0.000146

configd creates it based on configuration information. lookupd appears to be reading it (which surprises me).

Quite possibly OSX won't pick up changes made directly to resolv.conf, but you can cat /etc/resolv.conf to get a quick overview of what the current settings are.

Can also use scutil.
