The answer lies in the way the printing system translates the client-side PaperInfo ticket to the CUPS media option value and back again to a server-side PaperInfo ticket. In the case of a document formatted for the generic printer's US Letter, the printing system sets the CUPS media option value to na-letter on the client side. On the server side, the printing system scans your PM's PaperInfo array looking for a match. In your case it looks like the closest match is a PaperInfo ticket with a page rect {0.0 0.0 643.3 576.2}. I am surprised your PM registered a PaperInfo ticket for US Letter with such a large top or bottom margin.
