Yes - this was fixed in a later version of CUPS which was picked up
in 10.3.

Basically, internally CUPS provides a "Custom" page size listing
whenever the CustomPageSize attribute is present. Applications
set the page size to "Custom.WIDTHxLENGTH", which selects the
Custom keyword and sets some state with the selected size. The
CUPS API code then checks for this selection and emits the custom
page size code as needed. Older versions of CUPS (such as those
included in OSX 10.2.x) didn't special-case the DSC comment...
