Yes, the drivers provide basic grayscale printing (dot matrix
definitely brings back memories of print quality past... :)

Use the custom page size support to print the proper length page
for the forms.

It is a parallel printer. AFAIK there are no parallel cards for OSX.
So the plan is to hook it up to a LPR capable print adaptor or spare
linux box. Has anyone tried this?

A USB-to-parallel adapter may do the trick, although we've had a
lot of reports of problems specifically with the Epson and Keyspan
USB-to-parallel cables (Belkin seems OK so far) and 10.2; no
reports on 10.3 yet, but I still have an open Radar bug (#3388108)
on it that hasn't been responded to yet...
