I have seen this issue also, it doesn't happen when you for example
choose to output to an another external hardware like a Decklink card.
In my case it happen when I was outputting to "Built In Output".
Definitely some bug in QT.

Hello,
I have spent almost two days trying to track down a bug and I have
finally found that if I call Disposemovie (using QT 7.1.2, OS
10.4.6 or 10.4.7 Intel and PPC) my core audio aggregate device's
IOProc stops being called. This prevents me from playing back audio
properly.
The proof is in the shark system trace where it shows the CoreAudio
is calling through to the writeoutput data procedure and then
during a call to disposemovie a call is made to stop the hardware
and from then on my IOProc is not being called any more.
Does any one know about this or a workaround ?
The only thing I can think of is to buffer all my disposemovie
calls until the audio system is about to be tore down and then
dispose of them.
Any help greatly appreciated.
