This one is definitely not a fast-start movie. I use an Applescript that Brennan Young kindly provided here: <http://lists.apple.com/archives/quicktime-users/2003/Oct/msg00015.html>

As to the cause of your problem, I'm not sure, it sounds like you're going by the book.

You can turn standard movies into fast-start ones by calling them through a tiny reference movie - saves you reexporting/-encoding.
