I am happy with Liberum

Some of the reporting could be better, but the web interface is good and
easily customizable and the tie in to Active Directory for accounts is
great. Unfortunately it must be hosted on a Windows box, but the browser
interface is fine on Mac, PC and Linux and it is free and fast to setup. I
particularly like the ability to enter select solutions into a knowledge
base for users or tech to search through.
