Just some ideas... I always still use the embed tag in addition to the object tag. I always make a reference movie to use in the embed/object tag, and put in the same directory as the web page, instead of the rtsp URL. The reference movie can be made with QT Player Pro. File>Open URL of your streaming movie: rtsp://....., save as self-contained .mov. Click on it to make sure it opens your stream. There are other ways too. Now, not sure about mp4, we are still using quicktime as a "wrapper".

Our stream is working in QT7 from this code embedded in a webpage, but not QT6.5.2 clients (mac/pc). However, if you open the rtsp link in the player, it works fine on both platforms. It was encoded in Compressor 2 using the Quicktime 6 Streaming 800kbps preset.

This is standard Golive CS 2 code.
