That's exactly what it does. It leaves you with completely blank media.

That's "Initialisation" in Mac terms.

Under what conditions does the host generate this command?

Usually only in response to a Format request from the user using disk formatting software (Disk Utility in X, Drive Setup in 9 or a third party utility like HDT).
