Apple-as-shipped issues Scsi op x04 Format?  Eh? 
Cool.

Anybody know in what forms?  i.e. what does the whole
Cdb look like.  And what data goes out with it?

That's exactly what it does. It leaves you with
completely blank media.

That's "Initialisation" in Mac terms.

Usually only in response to a Format request from the
user using disk formatting software (Disk Utility in
X, Drive Setup in 9 or a third party utility like
HDT).
