Sorry for the multiple messages yesterday; I was having email problems... The crash was not in MIDIServer; it's in the AppleMIDIUSBDriver.plugin. I suspect it doesn't like it when the device is unplugged before the driver actually gets a chance to determine whether its a MIDI class device or not. To anyone who developed a driver starting with SampleUSBMIDIDriver: Your driver might suffer from the same problem. See Doug Wyatt's posting on this issue for more details.
