Of course I've responded to all "click here to remove xx.xx.xx.xx from the black list" requests,

That's the problem, most spam filters will put a high probability of spam when they see a line like this in the content. You should write something like most mailing lists are asking: « click here to confirm your registration » and « if this email was send as a error, please ignore this email ».

What I meant was that for all messages received by me (either as "failed to deliver" emails or as text in the mail log), that contained instructions on how to remove from the RBL, I followed the instructions. Our emails do not contain such text.

but some messages are just refused. I suspect that the IP addresses we inherited for our new colocated XServe were perviously used by spammers.

Check your IP in various RBLs to see if it's the case.

I have checked a few, including www.completewhois.com, and have only found one, and have requested its removal.

Does anybody have any suggestions on how to overcome this problem? The client is tearing his hair out. Is there a commercial mail service where I can send mail for it to take care of?

No solutions are perfect. Are you including images in the mails ? If yes, does the images are included as attachments in the email or are links (<img src>) to a Web site ?

All emails contain image references, but no included images. Which is better?
