And they really shouldn't. There's nothing special about Xephyr on
OSX. It's the exact same as on other systems... so your general
googling that turns up how it works on Linux should be the same as how
it works on OSX.
Are there some docs that you can point to that clarify what to do and
what not to reinvent?
This works for me (well, after removing /usr/X11/share/X11/xkb):
startx /usr/X11/bin/twm -- /usr/X11/bin/Xephyr :2
That works fine on the Mac, with a local Xephyr instance, running the
X-apps on the same box.

How, exactly, is your approach used to connect me to a remote box,
ensuring proper authentication etc?
You mean XDMCP? I can do this on my Mac:
/usr/X11/bin/Xephyr :1 -query 10.0.10.1
I have kdm running on my Linux box (which is 10.0.10.1).
