FWIW, I saw that happen on the first launch of 2.2.1 (after updating
from 2.2.0.1).  I quit and restarted X11, and haven't seen it since;
but there's something a bit bogus in there.

IIRC, I had managed to fat-finger my clicking such that a Finder window
was opening at about the same time that the initial xterm window came
up, and I suspected that there might be some race condition involved.
I didn't make any effort to reproduce it though.  Another theory is
just that there was some strange state left over from 2.2.0.1.
