xcode build out of sync



I'm seeing situations where I make very small changes to a c++ file, save, and hit command-R to run. The changes don't seem to take effect. This doesn't seem to happen if I hit command-R and it asks me if I want to save (ie I didn't save). The problem is usually fixed/ reset by doing exactly that: making a change and not saving, but letting xcode ask if it should save.

Is there something I don't understand about run/command-R, or have others seen this, or...?
