Jeff,

Depending on which OS you're using, there are 4 places to look:

1. Apple menu items folder---> Recent Servers -- trash all those items
inside.
2. System Folder----> Servers -- trash all those items inside.
3. System Folder----> Preferences----> Servers -- trash all those items
inside.
4. AppleShare Pref prefs (in OS 8.1 and earlier as I recall), trash it.

I have found that in 8.6 and later, simply trashing the items in 1 & 2 above
does the trick.
