Strictly speaking, the "item" (as in the HIToolbarItemRef) isn't created for
any window. Once that's created, the system can create the views we see in
the toolbar, but none of my code has any reference to a window. You can tell
once it's actually embedded in a window, though, Why exactly do you need to
know?
