Not because my application is was doing this, but because I kept
seeing other apps open windows in front of the application I was
using. At that time I think I was told it was fixed for Panther. It
was a long time ago, though, so I'm not sure of the details.
I did fix a problem like this in Jaguar or Panther, which caused
background apps to open their windows over a Carbon foreground app
when the foreground app only had floating windows open and no
documents open. But there seem to be other causes that have not been
fixed.
