When cleaning up the last bits of FSSpecs in our code, I am now wondering how I would replace FSMakeFSSpec properly. The documentation recommends FSMakeFSRefUnicode, but that does not allow me to create an FSRef from a volume number, directory ID and filename - which is what I have as a starting point.

Try this thread out from last week: <http://lists.apple.com/archives/carbon-dev/2007/Mar/msg00250.html>

Also note that you cannot create an FSRef from a non-existent file.
