The solution is to use kNavGenericSignature ('****') as the third
argument to NavCreatePutFileDialog() (that's the argument for the
creator code, not the type code). This gets rid of the top "default"
item in the Format menu.

Navigation Services Reference doesn't mention this crucial secret in
its description of NavCreatePutFileDialog() on page 18, but it does
mention it on page 100 where kNavGenericSignature is described.
