Not only the inconsistencies between Carbon and Cocoa, but the fact that it seems every major OS X release has included new UI elements and features which, from the user's point of view, represent new standards, but everyone outside Apple can only imitate them because the APIs won't be public until the next OS release... when the cycle starts all over again.

This is of course not to discount all the good stuff that does make it in. It's just that it seems like Apple is more interested in making a flashy entrance than in ensuring UI consistency, and this is just one of several things that give that impression.
