How are you adding the printers to each client, are you doing that by hand? ie: printer utility > add > browser etc. or using the command line to add them?
I don’t see how adding the printers into the directory is helping you if your still managing them via WGM – this seems to be where it gets broken, as mcx doesn’t set them up in a standard way...

I have experienced these same issues and have found it most successful to use LPR Queues and add the printers into Open Directory as outlined on page 27 of the 10.4 Print Services manual. This overcomes multiple printers being created and login speed issues. It also fixed an issue that on many logins the defined printer was not available to the user.

You can then manage them at the computer or group level in Workgroup Manager and deselect the "Allow user to modify printer list" so unassigned printers do not show. I am forcing out "
com.apple.print.PrintingPrefs.plist" to all groups so that users page setup is to A4.

One problem that I have had with the setup is that the LPR
queue often does not appear to start up correctly if you reboot the server. To overcome this I am having to religiously stop and start the print service after a system reboot. If I do not the client will be presented with a message that the host is unavailable or busy. This is occurring on 6 separate 10.4.2 servers at different locations so I assume it is a bug.

Print quotas and cover pages work correctly with this step for me. However I would love if someone could advise if there is a way to print user detail at the top of their document as Macintosh Manager used to allow instead of a whole cover page.

I've also tried to manage printers through Workgroup Manager but with that setup if I give access to a group to use certain printers, every time a user logs in it pulls all the printer files into the Printer Setup Utility's window or database but does not take them out when the user logs out. So it just keeps adding more printers every time a user logs in. I've had as many as 140 printers in this file before I discovered the problem which was causing extreme slow down in login times. If i don't manage the printers the printers stay put and only the ones on the client machines are available.
 --I have a similar setup and have never seen this. We use WGM on the student accounts to help control their access to printers. About the closest I have seen is that printers setup on the local admin account remain on their list until cleared. This works so well that many times when we had issues with 10.3.9 server students would simply delete their printers, and just log in and out to restore them.
