We've been having the same problem as Matt, and I am also unable to
determine the cause (and solution!)
This has been happening ever since we started using Tiger, maybe
10.4.5, and still happens with 10.4.11. MCX, network home
directories, Intel or PPC doesn't matter, and X-Serve Intel or PPC,
that doesn't matter either. We've done various hardware upgrades and
it keeps happening.
It's curious that the machine isn't really dead. I can SSH in, etc,
and the machine works fine, it's just that the login window is
waiting for something. And it seems there's no time-out in the login
window, (hint, anyone from Apple listening that can fix that!) so the
only way to recover is by holding the power button and restarting.
I've ravaged all the log files, and examined running processes and
don't find anything tell-tale. I've also tried lowering the LDAP
time-out to 1 minute, which has no effect.
I thought it might be related to the system clock, but that doesn't
seem right either. Although Safari bombs and urps if the system time
isn't right (again, anyone from Apple listening that can fix that!)
that doesn't seem to prevent logins.
Hmmm.....

What is
the hardware its running on? Are you using network home
directories? Are
you using an X Serve to host them and/or host MCX? If so what
version of
OS X is on the X Serve? Are you doing any kind of any Active Directory
integration?
