It depends on your application. Many of the popular applications from Apple are not document based. I.e. iTunes, iCal, Address Book, Mail, etc...



In particular, iTunes is not actually based on Cocoa at all :) It is written in Carbon. (Since there's a Windows version that needs to stay in lockstep with the Mac, it will probably stay Carbon, barring a complete rewrite.)

Well... I meant more conceptually. When you open iTunes you don't see any "document windows" per se. Same thing for iPhoto. These are clever browsers for libraries of data, but they don't have "documents".
