It seems to be related (maybe) to this statement in his console log:

CGImageSourceCreateWithData data parameter is nil

I have some screen shots from the user if they might help.

Ive reported something similar before, based on radiobuttons and the cocoa open file dialog...

are all of these drawing issues related?

does anyone have a clue what the problem might be?
