Presetting color temp before using PreCAL is simply to get you in the best possible range... its not an absolute necessity.


And, in terms of casually eyeballing, the "normal" sRGB preset 
looks the best.


Then that may be where you get your best results. With an LCD, not messing with the engineered presets more than necessary, and profiling it at those defaults is often the best solution.

either with the individual RGB controls or the brightness control 
before profiling?


Lower to brightness control if it makes it more comfortable, but don't mess with the luminance via the RGB Gains in an LCD.


Full brightness is a bit breath taking.


Then lower the brightness control before starting; assuming its really a backlight control...


- What gama should I use in Optical?


2.2., no reason for 1.8 these days...


I've always used 6500/1.8 on the 
Mitsubishi, and native/1.8 on the PB. I get confused about gamma. Do 
I see more shades of gray with higher gamma, or fewer?


You will lose more levels forcing a CRT with a native gamma of 2.2plus to 1.8 than using it at 2.2. Same for an LCD built to more or less emulate a CRT. I;d suggest moving on from 1.8 to 2.2 for all your devices. Things won't look any different in Photoshop, where gamma compensation via the profile takes place, you'll just lose fewer levels.
