Why can't you go to the Color Settings dialogue and under the pulldown for the cmyk working space go to the very top and choose custom cmyk.

In there you can pick the primaries, dot gain, UCR/GCR, black generation, under color addition as well as total ink limit etc. After making your selections the cmyk working space will say something like: SWOP (Coated), 18%, GCR, Custom. You can now pull the menu down again to "Save CMYK" and name and save that as an ICC profile.

I know that this is probably too simple and not sophisticated enough for the color gurus on this list but it is an excellent way to create heavy black profiles with small amounts of cmy ( controlled by UCA and K gen) for converting black and whites to rich black and whites.

btw it is also an excellent way to generate false RGB working space profiles different gammas for extremely light or dark RGB images. Saves tweaking the curves so much.
