Have you tried leaving off the refresh argument?

I have the same result. Also, the screen looks perfect in terms of refresh rate and resolution, it's just that I have no windows visible.

I noticed something new though. Apparently, an xterm is appearing, I just can't see it because there is solid grey over the top of everything.

I get the mouse icon indicating that I should set the location of the xterm, and the mouse changes to the i-bar icon when I'm inside the area where the xterm is, and I am able to execute commands, e..g I was able to su to root and reboot.
