Wow, it looks like it works. I copied over /System/Library/Keyboards
from opendarwin and ran:
startx -- /usr/X11R6/bin/XDarwin
and I get a window manager, including it's background image. I tried
window maker and twm. However, when I exit the window manager,
whatever the background is left there and the swirly icon appears, and
I'm stuck.
Yes I remember, that was exactly the same for me
So the problem is not with the hardware directly
If I try to run as my normal user, I get the default dimpled X
background, with no window manager. Before it starts, there is an
error message like: /tmp/.X11-unix should be owned by root.
I think this is a well-known issue from running X11.app and XDarwin but
I don't remember at all how to have it fixed !
Did you use XFree86-Darwin-ppc from GNU-Darwin ? Because on MacOSX,
you are expected to use XFree86-Panther

This is, using apples X11. If I login as root, start bash and do
startx I seem to get no window manager. The background is blue, and
there is an apple menu that does nothing.
In my .xinitrc I have:
twm &
xterm &
xterm
An xterm appears iconified, when I open it, I get the placement
cursor which does nothing if I click on the background, but places
the other xterm, if I click while the cursor is over the other
xterm. If I exit all xterms, I'm dumped out to a solid blue
background with the swirley wait animation, and I'm stuck.
I get the same result using startx -- -iokit, which I read someone
mentioning somewhere.

Have you tried leaving off the refresh argument?
I have the same result. Also, the screen looks perfect in terms of
refresh rate and resolution, it's just that I have no windows
visible.
I noticed something new though. Apparently, an xterm is appearing,
I just can't see it because there is solid grey over the top of
everything.
I get the mouse icon indicating that I should set the location of
the xterm, and the mouse changes to the i-bar icon when I'm
inside the area where the xterm is, and I am able to execute
commands, e..g I was able to su to root and reboot.
