No, it's definitely not the disc, it erases fine on Snow Leopard (and via drutil and Disk Utility on Leopard) and I just tried a brand new one with identical results. It looks like a Leopard bug to me. What version(s) of OS X are you using? It's a DVD+RW if that makes any difference. Tiger doesn't seem to be able to erase these at all (it reports something along the lines of "Media is not eraseable").

Another thing I have noticed is that 'drutil status' reports "A background format was started but is not currently running and is not complete." What that means I'm not sure but it sounds a bit suspicious. But then again, drutil always seems to say that when there is a DVDRW in the drive. Also, at the end of the erase, drutil briefly outputs a line of stars. Looks like some kind of progress indicator, but what is it doing? Something outside the DiscRecording framework, looks like. It can do other things that DR cannot do too, such as reading the ATIP. Why can DR not do this? Seems to me it is not getting any developer attention.

Anyway, the DVD erase issue is really bugging me. Anyone from Apple listening? This mailing list does not seem to be particularly active.

I'm surprised to hear that. I use DRErase in my software and I have no trouble erasing a DVD+RW. Maybe the disc has been erased too many times (40-50 times ?).
