I had similar difficulty recently. I finally found yet another old keychain entry, which when deleted allowed me to login to AKO.

My password is being rejected every time I try to log in to AKO via  
Safari. I deleted any reference in the keychain to this password, but  
still no luck. BTW I have no trouble logging on through FireFox.
