Step one: insert your CAC card before you launch Mail.

In my experience with OS X 10.4.x the buttons for encrypting and signing will not appear in new messages if you insert the CAC card after launching OS X Mail.

Step two: in a new message to the right of the signature pop-down there is a lock and a check mark in a star. The lock is to encrypt and the star is to sign.

When I put my CAC card into the reader, Keychain Access recognizes my card, and I'm able to unlock it with my pin.
