Some deployments modify the OWA authentication DLL.  E.g., MITRE's OWA deployment uses a custom auth DLL that presents me with additional way to modify features for the session.  Customizing the auth DLL tends to break non-browser OWA clients like Entourage, Mail, and Evolution.
 
Not saying that's what's happening to you, but it's possible.

Sounds simple enough, I followed the steps below and got prompted for my pin and it seems to connect but then not folders or mail is displayed.

Has anyone else seen this problem?

1) Launch Entourage 2008, choose 'Entourage' from the menu, then 'Account Settings'
2) Click on the 'New Icon'
3) When the 'New Account Dialog' appears, choose 'Exchange' for the account type (it's easier w/o the assistant), then select 'ok'
4) Under the 'Account Settings' tab, provide an Account Name, your name, your email address. Under Authentication, fill in Account ID, Windows  Domain and fill in the password field
5) Under Server information, Exchange Server, enter the following: https://webmail.yourorg.mil/exchange  check the box next to 'This DAV service requires a secure connection'
6) At this point, be sure your CAC is in and visible in Keychain Access.app
7) Select the 'Advanced Tab'
8) Under Client Certificate-based Authentication, select the email signing cert from your CAC card (this is the second cert for most)
9) Select 'OK' and you should be off and running.

Yes, Erik, I would also like the details of your Entourage 2008 setup. Thanks again for your help. It is much appreciated. This list is a fantastic resource for people like me who are not network experts. I find networking in general to be the most difficult aspect of computer administration. That said, I was able to get Back to My Mac working. I had to open the correct ports on my Firewall and limit access to just the IP addresses of my laptops. But that's another story for another list.

I have this working, but decided that MS Entourage 2008 is a MUCH
better solution for OWA email. If you want more gory details
