This was done on 10.5.4 so it's _possible_ this will not help, however:

During my trials and errors to get OWA access on my mac with my cac I
tried out admitmac software by thursby. Long story short after
installing and uninstalling their software I was unable to get my mac to
recognize the certificates on my card when I know that I used the
correct procedure to get it to work. I re-installed the OS and used the
exact same procedure as before to get it to work, except I did not
install and uninstall admitmac. So it may be that thursby's software
changes enough of the OS that it "breaks" the functionality once it is
uninstalled. Good luck!
