If you absolutely insist on doing this, then use the official Apple sample.
Much of the interesting stuff is in a file called "MoreSystemMenus.c".

But PLEASE PLEASE PLEASE don't do this. It's almost always bad human
interface, it creates a greater-than-usual maintenance and compatibility
risk for both you and Apple, and it won't work in Carbon anyway. Instead, do
tell us why you are tempted to mess with system menus, and maybe we can help
you avoid the hassle.
