Need example JNLP with working icons

I was wondering if any of you have an example JNLP where the correct icon is used. I can get the icon to show up in the dock but the created .app on the desktop still has the generic application icon. I have tried several variations of <icon> tags, images, and JVM (1.5 vs 1.6) to no avail. Does anyone have this working? Is it a problem with my install?

My system:

Macbook 2GHz Mac OS X 10.4.7

Java Web Start 1.6.0-dp
