I think three slashes in the URL is correct (if you're omitting the
host name) but QuickTime doesn't like that. I must have lost one in
the process of prepending the volume name and that's what suddenly
made it work.

Isn't file://Users/rolf/file.mov a malformed URL? I think that this
ought to get filed as a bug against QuickTime. Did you try other
well-formed URLs like file://localhost/Users/rolf/file.mov?
