It would be great if someone solved this problem and donated the code to SDL (www.libsdl.org). SDL already has some level of support for keyboard, mouse, and joysticks, but I don't see any support for d-pads on OS X.

This might work for you: HID Wizard from Z Sculpt.

It is an InputSprocket clone for OS X which also works (using InputSprocket itself) on OS 9. It is donation-ware, meaning that it asks the end user for a donation, so it probably isn't good for all projects. I thought I read that someone else was working on an InputSprocket for Cocoa, but I can't find the reference now.
