I know is not a pleasant read but the ARB_VERTEX_PROGRAM spec is the best at the moment. Get it via the opengl page at Apple. It helps if you have a good search facility on your text editor:)

Barring that have a look at the nvidia stuff. The syntax isn't that different that you couldn't figure it out.
