sendmail/AMS instructions

Hi all,

I've had a chance this weekend to revisit the instructions for getting sendmail to work right under OSX. Sorry I have been unable to look at it before, but I've been busy. What I've changed:

* virtusertable explanation and example so that you can get virtual hosts working with sendmail
* testing with 10.1.5 for sendmail (without AMS) and necessary adjustments
* rewriting and clarification of many parts, dividing document into sections


As always, please let me know if there is anything you think could be improved.
