My guess:

You are trying to process a file with the .html extension. In the configuration, Apache is set not to process .html files, and hence never process the includes. If you change the file name (and the include statement) to .shtml it will probably work. Oh, any you'll probably want to add index.shtml as one of the default pages as well.
