I was able to get mysql running and also get webmin successfully installed.
It's really a great tool. Also - I can successfully log into the php module
and look around, etc... But what I can't do is create a new database. I
get the following error:

Failed to create database : SQL create database phpbb failed :

But no further info. I checked the database directory and there was no new
directory created for the phpbb database I was trying to create - so truly
it was correct, database creation failed.

Have you actually configured mysql with account information? Did you read the large README file that accompanies the source?

The only clue I can come up with is that in the module config page the
option: "Path to MySQL shared libraries directory" is set to:
/usr/local/mysql/lib - I'm pretty sure that's incorrect but I cannot find
where this is supposed to be.

That would indeed be incorrect. Again, use locate to find these files. If the locate database hasn't been updated as part of the 'periodic' tasks, you may have to update it manually. See the man page for locate.

One other clue is that the webmin app defaulted all mysql stuff to
/usr/local when it was supposed to be /usr/bin/ - I corrected all the
others but cannot figure this one out.

As I pointed out, on most unix systems this is where the sysadmin generally would install it.

Anyone have any clues? I used the standard apple mysql installer (ie:
install, start/stop being the only options) to install mysql originally.

Again, just installing the product may not be enough, you will probably need to configure it. Again, see the mysql documentation for how to do this.
