Are you running anything except OS X Server 10.3 on this? I had this problem when I was running SpamCatcher and Communigate Pro. Disabling SpamCatcher fixed the problem.
I'd also have a look at the RAM. If you have AppleCare, I'm sure Apple would be very happy to help. I've found them to be quite useful.

i would suspect hardware. move the drive modules to another xserve and
let it run for a while and see if the issue occurs.
