Boot in single user mode or off the installer CD, mount the boot disk and check to make sure you don't have a stray mountpoint on the normal boot volume in /Volumes or /Volumes/bootvolume/Volumes.

Yes I forgot to say I found my differents volumes in the /Volumes. How can I prevent this to happen in the future ? I mean one security update, one reboot and I can be in the same trap again.
